1.7.16.5 Anti-Glomular Basement Membrane Antibodies

Anti-glomular basement membrane antibodies (mainly IgG, sometimes IgA), directed againsttype IV collagen in the glomerular basement membrane of the kidney, form immune complexes of autoantibodies that become deposited in the filtering system of the kidneys. The main function of the glomerular basement membrane (GBM) of the kidney is ultrafiltration of blood and type IV collagen is a typical component of the GBM. The formation of antibodies against the basement membrane of the filtering system results in complement activation, damage to the basement membrane, and kidney failure. The antibodies are seen in rapidly progressing glomerulonephritis and may also be seen in some ANCA associated vasculitides. The antigen is also present in the alveolar basement membrane of the lung and the involvement of nephritis and pulmonary haemorrhage is seen in Goodpasture’s syndrome. Anti-glomular basement membrane antibodies can be detected by ELISA or indirect immunofluorescence assay.
